Responsibilities

Support organizational strategy

Work in tight collaboration with practice manager and outreach manager/director to achieve organizational objectives

Drive implementation of OSH strategic initiatives

Follow direction from regional leadership

Coordinate and lead onboarding of new providers at the center

Model excellent medical care and success in metrics

Ensure appropriate evidence-based care keeps patients healthy and out of hospital

Understand, teach, and implement Oak Street's clinical guidelines

Provide highest quality preventative care

Educate providers on value-based full risk model of care
