Support organizational strategy

Work in tight collaboration with practice manager and outreach manager/director to achieve objectives: best care anywhere, unmatched patient experience, exceptional growth, great place to work, leader in DE&I

Drive implementation of OSH strategic initiatives

Follow direction from regional leadership (regional medical director or senior medical director) in collaboration with CMDs in region to implement priorities and reach objectives

Cascade both organizational information to care team members as well as escalate concerns/ideas to organization

Coordinate and lead onboarding of new providers at center

Model excellent medical care, implementation of care model, success in metrics

Ensure consistent robust OSH care model by ensuring preparation/executing daily huddles, patient care planning/managing, admission prevention, clinical documentation workflows

Provide appropriate evidence-based care that keeps patients healthy/out hospital

Understand, teach, implement Oak Street's guidelines at provider and center level

Support each care team and center for highest quality primary and secondary preventative care

Educate/providers ongoing support for value based full risk model of care

Assess/address root causes when metric goals not reached

Create/maintain culture coaching up/growth of providers/care teams

Listen feedback from care team members address concerns constructive problem-solving
