Responsibilities
Conduct in-home health evaluations lasting approximately 45 minutes
Perform medication and medical history reviews
Complete physical evaluations in patients' homes
Administer diagnostic tests such as spirometry and diabetic eye exams when ordered
Use an iPad with an intuitive clinical workflow for documentation
Build trusted relationships with health plan members
Provide education to help patients better understand their health
Answer patient health questions without rushing
Travel locally and within licensed states as needed
Conduct virtual visits when required
