Conduct 45-minute in-home health evaluations
Perform medication and medical history reviews
Complete physical evaluations
Administer diagnostic tests as ordered (e.g., spirometry, diabetic eye exams)
Use iPad and clinical workflow tools for evaluations
Provide referrals to health plans
Communicate findings to primary care providers
Conduct virtual visits as needed
Support comprehensive and personalized health assessments
Build trusted relationships with patients in home settings
