Responsibilities

Provide and direct provisions of nursing care to patients in their homes

Coordinate total plan of care with other health care professionals

Provide high-quality clinical services within scope of practice and infection control standards

Complete clinical nursing assessments per federal/state program requirements and payer needs

Ensure patient/client eligibility and medical necessity for services

Develop and revise individualized plans of care/service plans

Ensure plan of care frequency and duration meets patient needs
